When providing your personalization details, please enter:
1. Your last name in the format of "The (Last Name)s" *
2. Your established date (Optional)

Example:

* How to make your last name plural:
An apostrophe is not used to make your last name plural. For last names that end in s, x, or z, add -es to make the name plural. For all other last names, add -s.

Please note, the established date is formatted as "MM/DD/YY". Zeros will be omitted for a cleaner look. For example, June 12, 2021 will read as "EST 6/12/21". October 12, 2023 will read as "EST 10/12/23". The established date may be omitted if desired.
